We utilize skilled nursing and therapy services to help you meet your goals. Below are examples of the services we provide:

If your doctor prescribes short-term antibiotics, antifungals, or the common cardiac infusion drug Milrinone, Grey to Glory Home Health can assist. Our skilled nurses can administer these medications and teach patients and caregivers how to do so in the comfort of their own home. Additionally, we can provide your doctor with the necessary weekly lab monitoring.

Grey to Glory Home Health will manage any PICC lines, midlines, or implanted ports on a weekly basis if requested by your provider. We will collaborate with your infusion service to ensure you receive the best possible care. We also offer 24-hour assistance for any questions that may arise.

Grey to Glory Home Health is staffed by wound-certified nurses dedicated to providing the highest quality wound care. We collaborate with local wound care clinics, surgeons, and your primary doctors to determine the best treatment for your wound. Our team will teach you and your caregiver how to manage the wound, provide weekly monitoring (or more frequently if needed), and assist with ordering wound care supplies.

Additionally, Grey to Glory is proficient in Wound Vac changes, which can significantly aid in the speedy closure of your wound.

Many patients manage urinary complications, such as Neurogenic Bladder, with permanent urethral or suprapubic catheters, which require regular changes—monthly or even every two weeks. Grey to Glory Home Health can assist with these routine catheter changes, eliminating the need for frequent trips to the urological office. This service is especially beneficial when leaving home is a taxing effort.

Medication errors are a significant cause of hospitalizations among the elderly. With multiple visits to different providers, managing medications can become confusing. Grey to Glory Home Health can assist with this. Our nurses will collaborate with all your providers to obtain an updated medication list and can set up weekly pill boxes to ensure compliance. They will also teach you and your caregiver how to properly manage your medications and understand their purposes.

Once insurance no longer covers this benefit, Grey to Glory offers this service as a personal/private pay feature to continue on a more continuous basis.

Newly diagnosed with diabetes? Just found out you have congestive heart failure? Need to adjust your diet due to kidney failure? Grey to Glory Home Health is here to help. Our nurses will ensure that you and your caregiver are well-informed on how to manage your new or existing diagnosis, including necessary dietary changes, prescribed medications, and what needs to be reported to your provider.

We aim to prevent hospitalizations by identifying early signs of complications and making sure you know what to look out for as well.

 The primary goal of any home health agency should be to keep you out of the hospital, but not all agencies achieve this. Grey to Glory Home Health is committed to making a difference. We offer short-term nursing services to monitor wound status and therapists to initiate your exercise plan until outpatient services are appropriate. Our therapists are experienced in managing knee replacements, hip replacements, and other orthopedic surgeries, providing the tender love and care required for a smooth recovery.

 

Are you a patient on continuous Milrinone? Grey to Glory Home Health can manage that. Do you have a temporary chest tube placement? We can assist with that too. Our nurses are highly trained and ready to support you every step of the way. They will ensure you are well-stocked with the necessary supplies and help you feel comfortable managing these challenging situations in the comfort of your own home.

All our employees undergo thorough background screenings and are CPR certified. You can trust that you will receive safe, reliable, and high-quality care from our clinicians. We collaborate closely with you and your family to develop a customized care plan and manage it meticulously at every stage.

Your Choice is Valued

If you have Medicare and your doctor determines that you require in-home skilled services, you have the right to choose any Medicare-certified agency that serves your area. Home health agencies are certified to ensure they meet both State and Federal health and safety standards. Your choice should be respected, and you have a say in selecting the agency you prefer.

Please note that even if you are an uninsured client in need of skilled services, Grey to Glory can still provide care with a doctor’s order and a private pay option. We are one of the few agencies in the area offering services to uninsured clients

New to Home Health?
Here is what you can expect.

Once you and your doctor (or hospital/rehab discharge planner) have decided to receive in-home skilled services and have chosen Grey to Glory Home Health, you are ready for a visit. Visits are scheduled within 48 hours of receiving the referral, unless you request otherwise. At that time, one of our staff members will contact you to arrange an in-home assessment. This assessment, which takes approximately 1.5 to 2.5 hours, will thoroughly evaluate your physical and medical needs. We will then collaborate with your doctor to develop a comprehensive plan of care, including all necessary disciplines, treatments, and equipment.

Our staff will continue to make regular visits to your home to implement the plan of care, which is regularly reviewed by both our home health staff and your doctor. If your condition changes during any of these visits, we will coordinate with you and your doctor to adjust the plan of care as needed. Throughout our visits, we will educate you and your caregivers on various aspects of your care, such as wound care, home exercise plans, and disease management. We will also teach you and your caregivers how to properly manage medications, set up pill boxes, and recognize potential side effects, including whom to contact if they occur.

When you have achieved all the goals outlined in the care plan, and your doctor deems it appropriate, or if you are no longer eligible for skilled services, you will be discharged from Grey to Glory Home Health. Your doctor may recommend continuing treatment at an outpatient facility or through regular follow-up appointments with them or your specialists. If you require additional non-medical services (such as sitters, meal preparation, house cleaning, or bathing), please contact us to discuss transitioning to our Grey to Glory, Inc. (non-profit organization), which offers private duty services through volunteers.
